Essential buyer details
A wholesale order form should identify both parties and the order. Include the buyer's business name, contact name, email or phone, delivery address, order date and purchase-order number where applicable. The form should also show your own brand and sales contact details.

Order totals
Leave separate spaces for subtotal, shipping, tax and final total. A printable template may leave those values blank until the order is reviewed. If you automate calculations elsewhere, keep the accepted order confirmation as the authoritative commercial record.
Terms to place near the form
- Minimum opening order and reorder minimum
- Payment terms or payment timing
- Typical lead time
- Shipping-charge wording
- Damages or shortage reporting window
- Catalogue or price-list validity date
The form should summarise terms, not attempt to replace a tailored wholesale agreement where one is required.
Printed form or spreadsheet?
A spreadsheet is useful for repeat accounts and automated totals. A printable order form remains useful during trade shows, showroom appointments and buyer meetings where products are reviewed together. Many small brands use both: the printed form captures the initial order, then the figures are transferred into the normal order system for confirmation.
Avoid common order-form errors
- Do not rely on product names alone when variants are similar.
- Do not show prices in mixed currencies.
- Do not omit case packs where the buyer cannot order single units.
- Do not leave old prices on a form after updating the catalogue.
- Do not treat an unreviewed form as a final accepted order.
